When I wrote Healthcare’s Trillion Dollar Disruption, I recounted how a collection of senior executives assembled by Oliver Wyman representing every sector of healthcare foresaw a radical reshaping of the industry. They predicted that over the course of ten years major winners and losers would be created as one-third of the annual revenues of the industry shifted from one set of players to another.

More important than the revenue shift is who would be the catalysts for a much higher performing system as measured by the Quadruple Aim. In this list, I highlight the game-changers. Some may also be big revenue winners but that isn’t the point of this list. After all, there are plenty of organizations profiting from today’s wasteful system, so revenue is only one metric of success. Save Over $800 Million in Avoidable ED Visits According to New APCD Analysis! November 30, 2015 ED Cost Driver Spot Analysis A new cost driver analysis based on commercial health payer data in the Colorado All Payer Claims Database (CO APCD) shows that Colorado could save an average of $1,150 per visit and over $800 million annually if patients used a clinic or doctor’s office for non-emergent situations. The report also identifies staggering cost differentials between non-urgent health issues seen in an ER versus an outpatient setting such as a physician’s office or urgent care center: Common cold – $709 versus $114 Acute Bronchitis – $999 versus $126 Sore Throat – $929 versus $131 Backache – $1,337 versus $188 Headache – $2,010 versus $375 According to the 2015 Colorado Health Access Survey (CHAS), roughly 40 percent of Colorado’s ER visits occur for non-emergent reasons. Given the estimated 1.8 million visits to Colorado’s ERs annually, that equates to roughly 720,000 potentially avoidable visits and substantial cost savings. “If you have a true emergency, it’s obviously important to call 911 or go to an ER,” said Ana English, CEO and President for CIVHC. “However, this CO APCD analysis shows that patients and the health system in general could save a lot of money while still receiving the care they need.”

It can be downloaded here. Highlights of the findings from the “Diabetes’ Impact in Colorado” Fact Sheet for November 2015. More and more Coloradans are developing diabetes. We were one of the states that saw the most significant increase in the adult prevalence of diabetes between 2013 and 2014. From 2014 alone the prevalence of obesity in Colorado, a key predictor of diabetes, increased 33%. Of all Coloradans with diabetes, 25% are undiagnosed. Diabetes can be incredibly expensive. The total cost of care for patients with diabetes alone is $13,000 – about 2.3 times higher than those without diabetes. (CBGH and other data bases reflect that diabetics with one or more co-morbid conditions can easily be four or more times expensive.) An astounding nine out of ten of adults with diabetes had at least one other chronic health condition; 70% of diabetics have hypertension. Adults with diabetes were twice as likely to have been diagnosed with depression. Many diabetics are not seeking care. One in four adults reported not seeing a doctor because of cost in the past year. What this expressly means for every employer is this: If you focus only on one disease in 2016, make it diabetes. Two programs should be considered essential: the Diabetes Prevention Program and some type of self-management diabetic class – in addition, of course, to screening. The Diabetes Prevention Program (DPP) helps individuals with prediabetes and at risk for type 2 diabetes get the lifestyle training and support that they need to prevent the development of type 2 diabetes. In Colorado, over 2,000 individuals have participated in the DPP, and this number continues to increase. For more information about the DPP in Colorado, visit the CDPHE website.
